Community Home Care Management
Suburban Hospital offers the Community Home Care Management Program, a community-based approach that includes a complete in-home evaluation for persons aged 65 and over living in Montgomery County, Maryland. The program strives to enable individuals to live independently at home by arranging for their safety and care. Based on a medical model, the Community Home Care Management program team consists of a geriatric nurse practitioner and licensed clinical social workers. This team helps older individuals and caregivers develop an appropriate plan. Some services by the clinical social worker and nurse practitioner may be covered under Medicare and third party insurances if the individual meets criteria for reimbursement under Medicare. Services are usually covered under long term care insurance.
